Over 70% of our patients are Hispanic
females with children; few would have an option for health care without
CDM.

Our Class-D Pharmacy provides nearly 20,000
prescriptions per year to patients who would otherwise go without
medication.

Our goal is to become the medical home for over
2,000 regular patients.

The health and
wellness component of Central Dallas Ministries is Community
Health Services, which includes primary care medical
services; pediatrics, pastoral counseling, social work and a Class D pharmacy operated in partnership with Welvista.
After successfully operating its first health
clinic for many years, the program opened its second clinic at the
Preston Road Church of Christ in December 2003. That clinic is known as Christ's
Family Clinic.
The CoDE
(Community Diabetes Education) program was launched in 2005. It was created with the intention of offering education to patients on their disease at their education level. The program has been such a success that it has prompted other community clinics to replicate it in their clinic setting. The
results of this program, which are outlined here, are gaining
national attention in the medical world.
These services are
made possible through the strength of collaborative partnerships with
Baylor Health Care System, Inc., Texas Tech School of Pharmacy,
Parkland Health and Hospital System, the Dallas
County Medical Society and several other community
organizations, churches, and non-profits.
We look at our
patients holistically, trying to assist them in confronting both medical and
non-medical barriers that prevent them from getting healthy. Our goal
is to be the health and wellness home of our patients, to teach them
preventative care and measures to enable them to take control of
chronic diseases (such as diabetes and hypertension), treat their acute
health needs, and help them access more advanced care and support when
needed through care navigation.
