Transition Resource Action Center
TRAC
is a centralized assessment/intake and referral center for
transitional living services for youth exiting foster and juvenile care
in North Texas. This is the only such facility or
program in North Texas that exists to comprehensively address the needs
of this population. TRAC has numerous
partners that choose to leverage their resources to create a
one-stop-shop to provide intensive planning and wrap-around services in
a single location and case management system, using a small core
coordinating staff at TRAC
and some staff from collaborating agencies.
TRAC's
mission is to connect youth, ages 14-24, who are transitioning from
substitute care toward self-sufficiency, with support, planning, and
access to community services.
TRAC
is a collaboration of the Dallas community and federal, state and local
governments working together to create positive outcomes for children
and adolescents in the court system. Individuals from all levels of
government are working together with volunteers, social service
agencies and community leaders to get children out of the court system
and to keep them out.
